Q: Why is my libido still low even though my hormones look normal?

A: This one comes up in my clinic all the time — both with men and women. You finally get your labs done, your testosterone or estrogen looks "normal," but you still have zero sex drive. What gives?

Let's clear this up: libido isn't controlled by one single number on your lab report. It's more like a team sport — and when one player's off, the whole system feels it.

I. The Hormone Balance Piece

Testosterone: Even if your total testosterone looks okay, it might not be doing its job if your free testosterone is low or your SHBG (sex hormone binding globulin) is too high. That means your body has the hormone but can't actually use it.

Estradiol (E2): Estrogen is key for both men and women when it comes to mood, blood flow, and sensitivity.

It's not just about being "in range." It's about whether your testosterone and estrogen are in balance with each other and working for you.

II. The Other Libido Killers Nobody Talks About

1. Chronic inflammation — if you've got underlying inflammation (from autoimmune issues, gut problems, blood sugar swings, or extra weight), your body diverts energy away from reproduction. Your immune system basically says, "Sex drive? Not a priority right now."

3. Sleep issues or sleep apnea — most of your testosterone is made during deep sleep. If you're snoring, waking up tired, or have OSA, your libido will absolutely take a hit. Treating sleep apnea can be a total game-changer for both energy and desire.

4. Pain and chronic discomfort — it's hard to feel romantic when your back, joints, or head hurt every day. Plus, pain increases inflammatory markers and lowers dopamine — your brain's motivation and reward chemical.

5. Metabolic issues — diabetes, insulin resistance, or poor circulation affect blood flow (and that's crucial for arousal). Even mild metabolic dysfunction can blunt nitric oxide — the same pathway Viagra works on.

III. What Actually Works

Once we calm the inflammation and balance the hormones, libido usually starts to return on its own.
